The Ministry of Health and Long-Term Care (MOHLTC) has established a toll-free referral support line to help Ontario physicians to arrange for assessment referrals and consultations for patients requesting medical assistance in dying (MAID). This referral support line is operational as of June 6, 2016, pending the voluntary registration of willing physicians.

Physicians are asked to complete the survey and either submit it electronically to MAIDregistration@ontario.ca or by mail to:

TO ACCESS THE SERVICE:
Clinicians seeking assistance in making a referral for an initial assessment or for a second opinion are asked to call the toll-free number 1-844-243-5880.
Once it is confirmed that the caller is a practicing clinician in Ontario, MOHLTC will use its database of registered clinicians to make a suitable match to another clinician. Once a match is made, MOHLTC will provide the contact information of clinician seeking assistance to the willing clinician.
MOHLTC will consider a clinician's name and all information provided to be confidential, and will not disclose this information to anyone except to another clinician to the extent necessary to facilitate a referral, or unless otherwise required by law. No patient information should be provided to the MOHLTC.
